SOHT’s strategy supports equitable, team-based primary care, with people and caregivers involved in shaping services. Its published initiatives include developing the Primary Care Network and supporting digital tools such as eReferral and online appointment booking.

SOHT works with partners to support Ontario’s eServices Program, including eReferral and eConsult. These tools support communication between care providers.
