What is a health team?
An Ontario Health Team brings organizations together so people can move more easily between health and community services. SOHT is a partnership network, not a single clinic, a hospital or an emergency service.
Rooted in Scarborough
Approved in 2020, SOHT brings together 40 partner organizations. Its work centres on Scarborough and people in neighbouring communities who receive care here.
Partners include community health centres, primary care providers, hospitals, home care, mental health organizations and community support services. Residents and caregivers contribute to how care is shaped.
